I specifically authorise Olivia Teahan to develop a sexual wellness plan for me.
I understand that consultations and wellness plans are not intended to diagnose, treat, cure, or prevent any disease or medical condition and that these are not being tested for or treated.
I understand that Olivia Teahan’s services, consultations and wellness plans are not a substitute for psychotherapy or medical care. I understand that this service does not include psychotherapy, it is one-to-one sexual wellbeing consultation. (Psychosexual therapy will become available in late 2025.)
I understand that no guarantee or assurance of any specific result is given or implied.
I understand that Olivia Teahan encourages me to consult with a medical doctor before starting, stopping, increasing, or decreasing any psychological, sexual, nutritional or exercise habits, or prescription medication.
Due to the potential impact of biology, hormones, medical conditions and illnesses on my sexual wellness, I understand that Olivia Teahan encourages me to have periodic medical checkups especially if I have persistent symptoms, recurrent symptoms or if I do not seem to be responding to the recommended sexual wellness strategy.
Payment for Services and Products
Olivia Teahan’s services are not covered by any insurance plan.
All fees for services are due prior to delivery.
A 24 hour notice period is required for cancellation or rescheduling.
The client may terminate services at any time.
Payment can be made via Revolut @sxslainte or Paypal oteahan@hotmail.com unless otherwise agreed.
Consent
I have discussed any questions I have with Olivia Teahan and have had my questions answered to my satisfaction. By signing the consent form linked below, I state that I have weighed the risks and benefits of the consultation service with Olivia Teahan and have decided that is in my best interest to participate in this sexual wellness plan and give my consent.
